Pediatric & Adult CHD
- Moderate or complex CHD: Tetralogy of Fallot, Fontan circulation, transposition of the great arteries, systemic RV, Ebstein anomaly, truncus arteriosus, univentricular anatomy, coarctation of the aorta
- Simple CHD (ASD, VSD, bicuspid AV) with any of the following: symptoms, arrhythmia, ventricular dysfunction, cyanosis, aortopathy, or pulmonary hypertension
- Adults with CHD who have not had ACHD specialist follow-up in ≥2 years
- Adolescents (ages 12–18) requiring structured transition to adult congenital heart care
- New or uncertain CHD diagnosis requiring longitudinal planning
- Pre- or post-operative patients needing shared-care coordination
Pregnancy & Heart Disease
- Any CHD with WHO pregnancy risk class II–IV
- Fontan circulation, systemic RV, prior cyanosis, repaired TOF, aortopathy, or pulmonary hypertension
- Preconception counseling for women with known or suspected CHD
- Need for multidisciplinary delivery planning (ACHD + MFM/OB + anesthesia)
- Postpartum surveillance after high-risk cardiac pregnancy
Heart Failure in CHD
- New or progressive ventricular dysfunction on imaging
- Rising NYHA class, recurrent congestion, or increasing biomarkers
- Evaluation for ICD, CRT, LVAD, or transplant candidacy in the setting of CHD anatomy
Pulmonary Hypertension in CHD
- Suspected or established Eisenmenger physiology
- CHD with PH where shunt closure, vasodilator strategy, or catheterization interpretation is uncertain
- Pregnancy counseling in patients with PH or cyanosis
Aortopathy
- Aortic root or ascending aortic dilation in bicuspid AV, conotruncal defect, or known genetic aortopathy (Marfan, Loeys-Dietz, vascular EDS)
- Uncertainty about surveillance interval, surgical threshold, or pregnancy safety
Second Opinions
- Major surgical or catheter-based reinterventions
- Discordant imaging or uncertainty about intervention timing
- Complex pregnancy risk stratification
