We may use or disclose your PHI for treatment (including telemedicine consultations and referrals), payment (insurance billing), and healthcare operations (quality improvement). Other uses require your written authorization, which you may revoke at any time.

- Request corrections to your health information
- Request restrictions on how your information is used
- Receive a paper copy of this notice
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
