Why Transition Matters
Many young adults with congenital heart disease fall out of cardiac care after leaving their pediatric cardiologist. This 'transition gap' can last years or even decades, during which time silent complications may develop. A structured transition to an ACHD specialist ensures continuity of care and prevents dangerous lapses in monitoring.
When and How to Start
Ideally, transition planning should begin around age 12–14 with gradual education about your heart condition, medications, and the importance of lifelong follow-up. By age 18–21, a formal transfer to an ACHD cardiologist should be completed. Your pediatric cardiologist can help coordinate this handoff to ensure no information is lost.
