Why Preconception Counseling Matters for Women With CHD
For women with congenital heart disease, the single most important step toward a safer pregnancy is preconception counseling — a dedicated cardiology consultation that happens before you try to conceive. During this visit, your cardiologist comprehensively reviews your cardiac diagnosis, current ventricular function, rhythm status, and medication list; applies the modified WHO (mWHO) classification to estimate your maternal cardiac risk; identifies any interventions that should ideally be completed before pregnancy; assesses the safety of your current medications; discusses the statistical probability of your baby inheriting a congenital heart defect; and coordinates a plan for fetal echocardiography and obstetric surveillance during the pregnancy. The goal is to enter pregnancy in the best possible cardiac condition with a team and a plan already in place.
High-Risk CHD Anatomies and Pregnancy
Some CHD anatomies carry a very high risk of maternal cardiac complications during pregnancy. Women with the Fontan circulation, systemic right ventricle dysfunction, severe pulmonary hypertension or Eisenmenger syndrome, severe unrepaired left-sided obstructive lesions, or a history of peripartum cardiomyopathy face the highest risk and require the most intensive preconception assessment and pregnancy co-management. For some women, preconception counseling may result in a recommendation to defer pregnancy until an intervention can be performed, to consider alternative family-building options, or — in the highest-risk situations — a frank discussion of the maternal mortality risk. These conversations are best conducted before conception so that all options can be considered deliberately.
