Why Pulmonary Hypertension in CHD Requires Subspecialty Care
Pulmonary arterial hypertension (PAH) associated with congenital heart disease — often abbreviated PH-CHD — is one of the most complex conditions managed in adult congenital cardiology. Unlike PAH in the general population, PH-CHD arises from prolonged exposure of the pulmonary vasculature to increased blood flow, pressure, or both, driven by structural defects that create left-to-right shunts. Over time, this exposure causes irreversible pulmonary vascular remodeling that raises pulmonary artery pressures and vascular resistance — a process that ultimately leads to right ventricular failure if not recognized and treated early. Managing PH-CHD requires deep familiarity with both pulmonary vascular physiology and congenital heart anatomy, two domains that rarely overlap in general cardiology practice.
Eisenmenger Syndrome
Eisenmenger syndrome is the most severe form of PH-CHD. It occurs when a large unrepaired or inadequately repaired shunt — such as a ventricular septal defect, atrial septal defect, or patent ductus arteriosus — causes pulmonary artery pressures to rise until they exceed systemic pressure, reversing the direction of shunt flow. Deoxygenated blood now flows from the right side of the heart to the left, causing chronic cyanosis, erythrocytosis (high red blood cell count), and a high risk of stroke, arrhythmia, heart failure, and premature death. Eisenmenger syndrome is a contraindication to most surgical repair and requires lifelong specialized medical management, including PAH-targeted therapies, iron supplementation, anticoagulation decisions, and careful management of intercurrent illness, procedures, and pregnancy.

PH-Targeted Therapies and Hemodynamic Evaluation
Modern PAH therapy has dramatically changed the prognosis for patients with Eisenmenger syndrome and other forms of PH-CHD. Endothelin receptor antagonists (such as bosentan and macitentan), phosphodiesterase-5 inhibitors (such as sildenafil and tadalafil), and prostacyclin pathway agents are now standard components of treatment and have been shown to improve exercise capacity, quality of life, and long-term outcomes. Hemodynamic evaluation by right heart catheterization — assessing pulmonary pressures, pulmonary vascular resistance, and shunt physiology — is essential before starting therapy, adjusting dosing, or considering surgical intervention.
