    What Is Eisenmenger Syndrome?

    Eisenmenger syndrome is a severe, irreversible form of pulmonary arterial hypertension that develops as a complication of unrepaired or inadequately repaired congenital heart defects — most commonly large ventricular septal defects, atrial septal defects, atrioventricular canal defects, or patent ductus arteriosus. The chronically elevated pulmonary blood flow from these defects damages the blood vessels in the lungs over years, progressively raising pulmonary vascular resistance until it exceeds systemic resistance. At that point, the shunt reverses direction — deoxygenated blood bypasses the lungs and enters the systemic circulation, causing the characteristic bluish discoloration (cyanosis) of the lips, fingertips, and mucous membranes.

    Management and PAH-Targeted Therapy

    Although Eisenmenger syndrome is not curable, modern PAH-targeted therapies have meaningfully improved survival and quality of life. Endothelin receptor antagonists, PDE-5 inhibitors, and prostacyclin-based therapies — used alone or in combination — reduce pulmonary vascular resistance, improve exercise tolerance, and slow disease progression. Management also requires careful attention to erythrocytosis and iron deficiency, anticoagulation decision-making, avoidance of dehydration and high altitude, and comprehensive guidance on activities, surgeries, and procedures that carry special precautions in Eisenmenger patients.     How It Works

    Pregnancy and Eisenmenger Syndrome

    Pregnancy in women with Eisenmenger syndrome carries a very high maternal risk. The physiologic changes of pregnancy — increased blood volume, decreased systemic vascular resistance, and the hemodynamic shifts of labor and delivery — are poorly tolerated in Eisenmenger physiology. If pregnancy occurs, it requires immediate referral to a multidisciplinary team with expertise in PH-CHD, maternal-fetal medicine, anesthesia, and neonatology. Preconception counseling to present all maternal risks and discuss contraception is a core part of our care for women with Eisenmenger syndrome.
