Recognizing Advanced Congenital Heart Failure
Advanced or end-stage congenital heart failure is characterized by severe symptoms that persist despite optimal medical therapy — including shortness of breath at rest or with minimal activity, refractory fluid retention, recurrent hospitalizations, progressive reduction in cardiac output, and intolerance of medications that would otherwise help. In the ACHD population, this stage may manifest differently than in patients with acquired heart failure; for example, adults with Fontan circulation may develop protein-losing enteropathy, plastic bronchitis, or liver fibrosis as signs of worsening systemic venous pressure rather than the classic signs of left-sided failure seen in general cardiology.
Advanced Therapies and Goals of Care
At the stage of advanced congenital heart failure, the care team expands to include specialized heart failure cardiologists, palliative care clinicians, and transplant specialists. Available options may include optimization of guideline-directed medical therapy, inotropic infusions, ventricular assist device implantation as a bridge to transplant or as destination therapy, and cardiac transplantation — with or without concurrent correction of residual structural defects. Palliative and supportive care — focused on symptom control, quality of life, and goals-of-care conversations — is an important parallel track at this stage.
